Request an appointment to register a death
You should register the death within 5 days unless the coroner has requested a post mortem or an inquest and they will then advise you when the death can be registered.
Deaths are registered by telephone. To request an appointment please complete the details below. Once you have submitted the form you will be given information on what happens next.
The information you provide on this form will be used by Cornwall Council’s Registration Service to make an appointment to register a death over the telephone. It will not be shared with any other organisations.
Your information will be held securely on our password protected network and will then be destroyed in a secure manner in accordance with Cornwall Council’s Record Retention Guidelines.
