Step by Step guide to making a request for help

Decide which Early Help service you need. Then use the links below to download the correct form. If you are unsure use the general Request for Help form.

The general Request for Help form for the following services or if you are unsure which service you need:

  • Early Years Inclusion Service. (NB all requests for EYIS must include the areas of developmental delay for the child)
  • Family Group Conferencing
  • Family Support
  • Health Visiting (over 2 years)
  • Parenting Support
  • Portage
  • School Nursing
  • Support Change in Partnership (SCIP)
  • Targeted Youth Support
  • Video Interaction Guidance

The Neurodevelopmental Assessment Referral process has further information. This includes assessments for:

  • Autism Spectrum Disorders (ASD)
  • Attention Defecit Hyperactivity Disorder (ADHD)
  • learning disabilities

  • Developmental Coordination Disorder
  • Neurodevelopmental Assessment Referral (including Autism Spectrum Disorder (ASD), Attention Deficit Disorder (ADHD), Tics, Learning, sensory and coordination difficulties)

Step 2

Complete the Request for Help by filling out as much of the form as possible. Please ensure that the following are fully completed:

  • personal details
  • consent
  • the reason for your request
